Use this template when you are interviewing someone and need to obtain their consent to:
- Record them on video or audio,
- Take their photo,
- Use photos or videos supplied by them, or
- Take or keep a written record of their interview responses.
The template also ensures you obtain their agreement on some important legal issues, including acknowledgement that you are the owner of the photos and recordings and that you can freely use or edit them for business purposes.
